Thank you for your interest in our Club.

Please fill out the Application Side of the form completely as possible.

Two KKC members must fill out the Sponsor’s Side and return it to the Membership Chairman listed on the form.

An applicant must attend three (3) meetings before being voted on by the membership in attendance at that meeting.
